Glukokortikoid i kombination med langtidsvirkende β2-agonist og langtidsvirkende antikolinergika (ICS/LABA/LAMA)
Samtidig anvendelse af både langtidsvirkende antikolinergikum (LAMA) og inhalationssteroid/langtidsvirkende β2-agonist kombinationen (ICS/LABA) betegnes trestof-behandling (eller triple-behandling).
Man kan anvende trestof-behandling hos patienter med KOL i stadium E og fortsætte, hvis behandlingen har klinisk effekt. Trestof-behandling er indiceret til patienter med tendens til eksacerbationer og virker bedst ved et eosinofilt tal i blodet på over 0,3 x 109 celler per liter ved at reducere antallet af eksacerbationer og mortalitet sammenlignet med behandling med LABA/LAMA (3032) (3190) (3191) (4445) (6200) (5410) (6201).
Kombinationen mometasonfuroat (ICS) med indacaterolacetat (LABA) og glycopyrroniumbromid (LAMA) er blevet undersøgt til patienter med svær astma, som ikke var tilstrækkeligt kontrolleret på en kombination af højdosis ICS og LABA og har haft 1-2 eksacerbationer det seneste år. Tillæg af glycopyrroniumbromid medførte forbedring af lungefunktionen og reduktion af eksacerbationsfrekvensen (5179).
Også kombinationen beclometason, formoterol og glycopyrriniumbromider er godkendt til vedligeholdelsesbehandling af astma hos voksne.
